Use the online form below or call us during office hours. New patient appointments are typically available within 3 to 5 business days (illustrative).

We send a secure patient portal link 48 hours before your visit. Completing forms in advance means more time with your provider, not at the front desk.

Bring your records
Any prior medical history, current medications, and insurance cards help us care for you well from the very first visit. We can also request records on your behalf.

Your first visit is unhurried - we want to understand your health history, your goals, and how we can best partner with you over the long term.
